DIS Disability Online application
Keep in mind that we are just applying for a Disability Identification Card (ID Card) online. If you have enough data to prove yourself as a disabled person then you will be selected. So before starting an online application you must have the below documents.
- Bangladeshi NID card
- Valid Mobile number and Email address
- Medical files
Go to this link - https://www.dis.gov.bd/SurveyForm/OnlineApplicationAfter visiting the dis official webpage you will get a popup notice. Read that notice carefully and click the agree button for start submitting information. You have to type your personal details like name, address, Ethnicity, age etc.
Complete below 3 sections for your final submission. For your help I attached all section information.
Section 1: In this section you have to submit your gender, merit status, region, reason of disability, educational information etc. Type all the information correctly and go to the next section. If you already receive a government incentive then confirm that too.
Section 2: There are only two options to select, One is for your occupation and another is your yearly income amount. Click next…
Section 3: You have to ask lot’s of questions regarding your disability. Answer properly for final submission. After completing all 45 questions you have to upload your signature. Click the submit button and next page you will be able to download your Application copy.
Now you have printed a copy where you can see your local dis office address and one officer name with a phone number. Contact that officer and give your application copy.
Put down your application number to check your application result online.
